In brightfield: Red blood cells displaying an off-centre white spot. May be observed on single RBCs or on RBCs in rouleau stacks. Ensure that the membrane of the cell is intact. Keratocytes may look similar, but their membranes are not intact and it looks like a bite has been taken out of the RBC. It is clear that the white spot is in fact on top of the RBC (and not a part of the cell) when the fine focus is adjusted. RBC fermentation is usually not visible in darkfield. In advanced cases it may be visible as white spots on RBCs.

Medical Perspective
Conventional medicine only recognizes localized Candida infections, such as oral and vaginal thrush, Candida of the nail (onychomycosis) and skin. The condition commonly referred to as “Candida” by alternative medicine, which is seen as a subclinical infection based on an overgrowth of Candida in the gastrointestinal tract, is not recognized by conventional medicine.

Implications
- These white spots are associated with increased fermentation in the blood and are most often seen in association with Candida albicans overgrowth and/or blood sugar imbalance.
